Using a life course model of aging well with intellectual and developmental disabilities (IDD), this presentation focuses on the needs of adults with IDD, the social-environmental context of their lives, and strategies for supporting them. As they age, they experience chronic and other health conditions, often at earlier ages than the general population, and changes in family caregiving and support, especially when parents are no longer able to provide care. The discussion will provide examples of policies and practices that address these challenges. First it will address strategies to reduce health disparities and promote health of adults aging with IDD. Secondly, it will provide examples of family support interventions, including a person centered and family centered approach to future planning. Finally, it will discuss ways to bridge the fields of aging and disability to improve policies and practices that promote healthy aging for adults with IDD.
